YLHS 2023 Men's Lacrosse Summer Camp

Dates: July 10-13th and 17-20th, 9-11am every day
Location: YLHS High School Stadium
There will be a pizza party down on the field for the last day of Camp.

PYLUSD Summer Athletics Camp Information 

​

REMINDER:  You will need a current physical clearance on file with the school BEFORE you participate in YLHS's Men's Lacrosse Summer Camp.

​

STEP 1:

  • District Registration and Payment for Summer Camp - [NOW OPEN]

    • Payment By Mail: (Check, Cashier’s Check or Money Orders Only)

      • Print completed registration from Family ID and attach payment (No Cash).  Please ensure that on your check, cashier's check or money order, there is:

        • A phone number where they can reach you with any questions

        • Name of child the payment is for

        • The school name and sport 

    • Payment In Person (preferred): (Check, Cashiers Check or Money Order Only)

      • Bring in copy of completed Family ID registration 

      • Attach payment (No Cash)

STEP 2:

  • Family ID Online Athletic and DMV Registrations

    • Follow instructions and create an account on Family ID or login with last year’s credentials. Family ID Instructions Here

    • Students with Valid Driver's Licenses can also complete the Online DMV/Risk Management Registration in Family ID

STEP 3:

  • Fill out “PLACENTIA-YORBA LINDA UNIFIED SCHOOL DISTRICT SUMMER HIGH SCHOOL SPORTS CAMPS REGISTRATION” Form Here

STEP 4:

STEP 5:

  • Turn in to YLHS Main Office prior to Camp Start Date of July 10th:

    • YLHS Registration form

    • PLACENTIA-YORBA LINDA UNIFIED SCHOOL DISTRICT SUMMER HIGH SCHOOL SPORTS CAMPS REGISTRATION

    • Paper copy of front and back of health insurance card.

    • Original completed PYLUSD Pre-Participation Sports Physical Form
